| Terms of Reference | At least one award, valued at a minimum of $10,000, will be granted annually in any term to an undergraduate student who meets the following criteria: - is enrolled in their first year in a program within the Faculty of Health Sciences; - is registered in a minimum of 9 units of standard graded courses at Simon Fraser University; - is in good academic standing; - has interest to pursue a career in medicine, health care, public health or health & wellness; and - has demonstrated volunteer service in helping others. Applications must include: - a one-page personal statement from the applicant outlining: - their interest to pursue a career in medicine, health care, public health or health &wellness and what inspires them to help others; - a CV outlining their volunteer service; and - one letter of reference from an individual in a leadership role, who can verify the applicant�s volunteerism in support of helping others. This award is renewable for up to 3 additional years, if the following conditions are met: - the Faculty submits a renewal nomination to Financial Aid and Awards for each of the award renewal years; - the recipient continues to be enrolled in a program within the Faculty of Health Sciences and is pursuing their first undergraduate degree; - the recipient enrolls in a minimum of 9 units of standard graded courses in the term of renewal; and - the recipient is in good academic standing. If the recipient fails to meet any of the above renewal requirements in any given year (Summer, Fall or Spring terms), the recipient will become ineligible to receive future award payments. This award cannot be deferred to future years. The award will be granted by the Senate Undergraduate Awards Adjudication Committee upon the recommendation of the Dean, Faculty of Health Sciences, or their designate. |
